Reminder: Deadline to receive flu shot is Nov. 19 for WVU Medicine

The WVU Medicine flu shot campaign is underway. Employees, physicians, licensed independent practitioners, residents, volunteers, students, contractors, and vendors throughout the organization are, once again, required to receive the vaccine, unless they have an approved medical or religious exemption. The deadline to receive the annual flu shot is Wednesday, Nov. 19.
The flu shot is available in Employee Health, located on the second floor of J.W. Ruby Memorial Hospital. The office is open weekdays, 6:30 a.m.-4:30 p.m.
Some reminders:
- You must complete the ReadySet flu survey prior to receiving the flu shot.
- If you receive your flu shot from a provider outside of Employee Health, proof of the immunization must be provided to Employee Health in order to be compliant with the requirement for mandatory vaccination. You must send your flu immunization record to Employee Health by Wednesday, Nov. 19. Documentation can be uploaded to ReadySet.
- If you are requesting medical or religious exemption for the flu vaccine, you will need to submit an exemption form through Workday. The deadline is Wednesday, Nov. 5. For those requesting a medical exemption, your primary care provider will need to sign a medical exemption form.
