Advanced Primary Care Innovation Center
The WVU School of Medicine Department of Family Medicine established an intradepartmental Advanced Primary Care Innovation Center. The Department has a long tradition of clinical and academic training, including medical student education, residency teaching, and fellowship training. The Department has demonstrated sustained leadership in primary care delivery innovations and practice transformation since 2012. The Department's primary practice site was the first clinic at WVU Medicine to receive the National Committee for Quality Assurance’s (NCQA) PCMH highest level 3 recognition in 2014. Our site continues to be nationally recognized with this award receiving recognition for 2019-2020. The dynamic team of clinicians and leaders in the Department has piloted numerous care delivery approaches that have spread throughout the healthcare system including nurse-led annual well visits, chronic care management (CCM), embedded nurse case management and others.
