This page contains the forms that are used in the Biochemistry and Molecular Medicine graduate curriculum.  These forms are also available for download from the Health Sciences Graduate Education site (Click Here) with the exception of the Committee Meeting Evaluation form, which is unique to Biochemistry and Molecular Medicine Graduate Program.  All forms must be typed.  In all cases, the student should submit a copy of the completed form with the committee members’ signatures to the Health Sciences Graduate Education Office (Ground Floor, Erma Byrd Biomedical Building).  The Assistant Vice President for Graduate Education will sign the form and it will be uploaded electronically into the student’s Degree Works academic file.

When you need it: This form should be completed by the first semester of year 2.

What it is: The list of faculty that serve on the student's advisory committee and their signatures.

Change of Ph.D. Committee Membership Form

Changes in the committee: To make a change in committee membership, a request is submitted to the HSGPO. This request must be signed by the committee chair, the student, and both the original faculty member and the substitute faculty member. Only one substitute is allowed and this individual must represent the same academic discipline or specialization. All committee members must be present at the final dissertation defense.

Plan of Study Form

When you need it: After 30 hours of coursework (including research credits) for Ph.D. students.

What it is: This form lists all the courses the student has completed, or plans to take, the degree that the student is applying for, and the title of the student's dissertation. It requires the signature of all the student's committee members.

Use this form to submit minor changes to the Plan of Study. If major changes are required, students must re-submit and obtain approval from their advisory committee using the Plan of Study form above.

Advisory Committee Evaluation Form

When you need it: After each Advisory Committee Meeting, including the defense of the dissertation proposal.

What it is: This form is a specific requirement of the Biochemistry and Molecular Medicine Graduate Program. The first portions of this form are to be completed by the student and given to the advisory committee 2-3 days prior to the meeting. The last portion is completed by the student's advisor after the committee meeting. It is the responsibility of the student to have the form signed by all the committee members. This form is a record of the student's progress during their course of study.

Doctoral Qualifying (Preliminary) Examination Form

When you need it: After the student successfully passes their written qualifying exam in June of the second year, this form will be generated by the exam chairperson.

What it is: This document certifies that the student has passed the written qualifying exam.

Doctoral Research Proposal Defense

When you need it: The student brings this form to their dissertation proposal defense. The student should refer to the Biochemistry and Molecular Medicine Handbook for how to prepare the dissertation proposal.

What it is: This document certifies that the student has passed their proposal defense and that the student is now a Ph.D. candidate.

Predoctoral Fellowship Biosketch Form

When you need it: The student will submit a biosketch with their dissertation proposal.

What it is: A resume of the student's scholarly and academic activities.

When you need it: This guide is used in preparing the student's dissertation proposal.

Individual Development Plan (IDP) form

When you need it: When you need it: All students in the BMS PhD, MS, and MD/PhD programs, and any students whose stipend is paid by an NIH grant, must complete the IDP annually and review it with their mentor.  There are separate forms for first year, second year, third year plus, and MD/Ph.D. students.

What it is: This form is a questionnaire that helps the student to assess their skills and abilities and to identify their career goals.

Application for Graduation and Diploma

When you need it: During the first 2 weeks of the semester in which you plan to graduate. Note: you must be a registered student in order to defend your dissertation and graduate.

What it is: Indicates your intention to graduate and allows you to indicate how you wish your name to appear on your diploma. Requires a fee. If the degree is not actually earned during that semester, the student must notify the Health Sciences Center Office of Graduate Education (HSGPO) and submit a new Application when registering for the term in which completion is again anticipated. A second fee will not be necessary.

Shuttle Sheet Request Form:

When you need it: 3 weeks prior to the student’s dissertation defense.

What it is: This forms indicates that the student plans to graduate soon. Upon receipt of this form, the Health Sciences Graduate Program Office will check the student's records to make sure that they have met all the requirements for graduation except for the final dissertation defense. In addition, the Health Sciences Graduate Program Office will arrange to have the student's defense seminar posted in the Mountaineer Spirit. Posting of the defense prior to its occurrence is required by the Graduate Council. All advisory committee members must sign this form.

When you need it: The student brings this form to their dissertation defense. Within 24 hours after your defense you must return this to the Health Sciences Graduate Program Office.

What it is: This form certifies that the student has successfully defended their dissertation. It is signed by the advisory committee.
