Program Educational Purpose

  1. Analyzing various patient populations to identify unmet needs and/or disparities in care, assess risk and propose actions to improve outcomes.
  2. Develop skills to teach about population health competencies to primary care teams, students/residents and other stakeholder. 
  3. Acquire applicable knowledge and experience regarding Patient-Centered Medical Home (Advanced Primary Care) models of care and NCQA recognition program requirements.
  4. Learn to implement non-traditional approaches to care delivery in primary care (examples include telemedicine, team-based models of care, integrated behavioral health, nurse case management, and collaborative pharmacy practice).
  5. Develop skills for leading and facilitating practice-based quality improvement efforts in primary care settings.
  6. Develop faculty skills in teaching, precepting, supervision/feedback, leadership and scholarship.
