COURSE DESCRIPTION:

The purpose of the Subinternship is to provide an advanced educational experience in the management of patients requiring inpatient hospital evaluation and care. Students will acquire and demonstrate the skills, knowledge and attitudes required of a subintern with responsibilities, under supervision, for the total care of the patient. This includes increasing experience in history taking, physical examination, formulation of a list of problems, and plans to solve these problems, and the execution of diagnostic measures and treatment measures. This also includes gaining experience working with patients in a complete physician's sense, as well as gaining experience in working with peers, colleagues and allied health personnel. By becoming a part of this educational team, further knowledge will be gained.

The course consists of four weeks hospital experience. You will be assisting residents and faculty in the care of our patients. In general, you will have experience on both the inpatient Labor and Delivery unit as well as with the inpatient gynecology service during your four week period.

OBJECTIVES:

Patient Care:

  • Demonstrate proficiency in history taking and physical examination
  • Development of differential diagnosis and management plans
  • Manage obstetrical and gynecological patients admitted to the hospital
  • Perform assigned procedures appropriately and professionally under appropriate supervision

Medical Knowledge:

  • Demonstrate competence in knowledge of common OB/GYN disease processes
  • Recognize low and high risk obstetrical complaints and the management of these issues
  • Recognize common gynecological conditions and their management
  • Describe principles of antepartum, intrapartum and postpartum care
  • Describe principles of preoperative and postoperative gynecologic care
  • Describe the technical skills necessary for surgical care
  • Describe the various gynecologic surgical procedures and know their expected outcomes and complications
  • Describe spontaneous vaginal delivery, operative delivery and cesarean delivery
  • Describe fetal heart rate monitoring in intrapartum obstetrical care

Practice-Based Learning:

  • Practice appraising and assimilating scientific evidence and improving patient care based on constant self-evaluation and lifelong learning
  • Apply knowledge of study designs and statistical methods to appraise evidence based literature
  • Demonstrate self-directed learning skills that allow students to take personal responsibility for learning current knowledge as well as lifelong learning
  • Participating in morbidity and mortality conferences, journal club and other lectures

Communication Skills:

  • Demonstrate courteous and compassionate communications with patients, family members, colleagues, and staff
  • Demonstrate how to optimize patients’ status prior to surgical procedures
  • Demonstrate well organized and concise patient presentation
  • Demonstrate well written notes containing appropriate patient-centered information

METHODS TO ACHIEVE OBJECTIVES:

Educators, including the Directors and designated preceptors, are charged to provide ongoing, formative feedback designed to maximize student learning, skill acquisition, and improvement during the clerkship. A formal mid-rotation formative feedback session will also be conducted between the Directors or their designee and the student to further promote learning and development. Topics addressed will include areas of concern, areas of mastery, and the Core Competencies. Care plans are reviewed and critiqued by the Director. The student’s acquisition of or failure to demonstrate the required competencies and learning objectives will be assessed by direct preceptor supervision. Failure to meet the level of proficiency expected at the student’s level of training for any one of the six Core Competencies may result in failure of the entire clerkship. The final summative narrative evaluation will address the student’s ability to successfully demonstrate the competencies and learning objectives across the Core Competencies.

EXAMINATION PROCEDURES AND EVALUATION CRITERIA:

Grading criteria

You will meet with the Sub-I director and/or coordinator on the first day of your rotation. During this Orientation, you will review Sub-I performance expectations, call schedule and inpatient team contact information.
You will also receive a copy of the Sub-I rotation competencies and the clinical evaluation form. Based on input from your educator evaluations, a final evaluation will be submitted to Student Services upon completion of your rotation.

To pass the clerkship a student must do all of the following:

  • Earn evaluations and narrative comments that reflect the student at least "Meets Expectations" in EACH of the 6 core competences.
  • Completion of all required activities including participation in daily rounds, participation in lecture series, grand rounds, morbidity and mortality conference and journal club conference, as well as completed required note writing and contributions to patients' EMR.
  • Give a short presentation on the topic of your choice at the end of the rotation.
