Transition of a Hospitalist Patient through Discharge

PRE-REQUISITE: Students must have completed the WVU Internal Medicine Sub-internship prior to taking this rotation.

OBJECTIVES:
Students will be introduced into a career of hospitalist medicine including the similarities and differences to academic wards in relation to patient care, time management, communication and knowledge acquisition. Students will also pay close attention to the discharge process, playing a role in the transition of a care of patient from hospital to primary care and outpatient follow up.

Patient Care:

  1. The students will be able to distinguish a career in hospitalist medicine, including recognizing similarities and differences to academic wards in relation to patient care, time management, communication and knowledge acquisition.
  2. The students will be able to manage the transitions of care for patients admitted on an inpatient service as they are discharged. This will including coordinating follow ups, managing changes to plan after discharge, and identifying and fixing errors that have occurred.
  3. The students will be able to identify healthcare disparities and barriers to discharge for hospitalized patients, and evaluate for improvements that can be made to the discharge process.

Medical Knowledge:

  1. The students will be able to demonstrate knowledge of the hospitalist model and its role in the hospital and national healthcare systems.

Practice-Based Learning:

METHODS TO ACHIEVE OBJECTIVES:

  1. Will rotate similar to hospitalist, completing I -week on and I-week off schedule throughout the 4-week.
  2. Students will rotate with Medicine Hospitalist I (MHI) and MH2 teaching services on Weeks 1 and 3 of the rotation functioning as an intern under the supervision of the resident and attending physician on the service.
  3. Students will be responsible for clinical evaluation of patients admitted to the hospitalist service. Duties include writing orders for diagnostic and therapeutic measures (cosigned by resident or attending), writing daily notes, coordinating and communicating with consult services, and patient education.
  4. On off-weeks, weeks 2 and 4, will spend two days in TCC clinic.
  5. Students will rotate in the TCC clinic functioning as an intern under the supervision of the faculty attending on TCC clinic for the provided days.
  6. Students will be responsive for clinical evaluation of 1-2 patients at discharge follow up, some of which they evaluated while on hospitalist. Duties include writing a discharge follow up note, patient education, and coordination of further follow up.
  7. Students will also review the patient's discharge summary with a grading rubric provided by the TCC staff to evaluate its accuracy and identify any barriers or errors that occurred in discharge.
  8. On off-weeks, weeks 2 and 4, students will write a 2-3 page reflection on the following: During week 2, students will reflect on their first week on hospitalist. They will be required to write a 2-3 page reflection on a healthcare disparity that they recognized while on the hospitalist rotation. This will include a review of the presented patient, literature review on the disparity, and a potential action plan to prevent a reoccurrence. During week 4, students will reflect on the discharge process. They will be required to write a 2-3 page reflection on an encountered barrier or error in patient discharge that occurred during or after hospitalization. This will include a review of the presented patient, literature review on the topic, and a potential action plan to prevent a reoccurrence.

EXAMINATION PROCEDURES AND EVALUATION CRITERIA:

Formative Assessment:

Will be given in the form of face-to-face feedback of daily observations for encounters and assessment by supervising faculty.

Summative Assessment: 

Summative assessment will occur via supervising hospitalist faculty by completing an objective based assessment via e*Value. Students will be required to complete both 2-3 page reflections and turn in to receive credit. A narrative will be included within e*Value.
